What You’ll Need to Replace your Vehicle Title in Utah

  • Application For Utah Duplicate Title (Form TC-123)
  • Vehicle Identification Number (VIN)
  • A copy of the most recent registration
  • License plate number
  • Proof of identity
  • Proof of ownership
  • Non- refundable Replacement fee of $6.00

 

What You’ll Do to Replace your Vehicle Title in Utah

  • If processing by mail:
    • Step 1: Complete an  Application for Utah Duplicate Title (Form TC-123)
    • Step 2: Attach copies of all the required documents
    • Step 3: Make checks or money orders of $6.00 payable to the Utah State Tax Commission (USTC)
    • Step 4: Mail it to Division of Motor Vehicles Mail and Correspondence

    • Step 5: You will receive the replacement by mail after a week

 

Caveats and Exceptions to Replacing your Vehicle Title in Utah

  • An owner of the vehicle must sign the title
  • If there was a lien on the vehicle, you may need to receive permission from the lienholder to have the replacement title issued
  • If your mailing address differs from the street address that is associated with your title, be sure to fill out section 1 of the application
